Before beginning your award nomination form, please check that you have the following information and supporting documents:

  • Nominee Information (person being nominated for the award):
    • Nominee’s IEEE Membership Number (all nominees must be ITSS members)
    • Nominee’s Professional Affiliation (place of employment, department, position)
    • Nominee’s Mailing Address
    • Nominee’s Educational Background
    • Nominee’s Professional History
  • Award-related Information
    • Proposed Award Citation (see examples here)
    • Nominee’s specific Technical, Professional, and Leadership Accomplishments and Contributions relevant to the award
    • Impact of the Nominee’s work (relevant to the award)
    • Nominee’s Involvements in the ITS Society
  • Reference Letters – all award nominations require THREE Letters of Reference. The Letter of Reference Template can be found here.
    • Name of each reference
    • Email address of each reference
    • IEEE Membership Number of each reference (if they are an IEEE member)
  • Nominator Information (person submitting the nomination):
    • Nominator’s relationship with the Nominee
